Established in 1979, the Illinois Department on Aging’s Community Care Program helps senior citizens, who might otherwise need nursing home care, to remain in their own homes by providing in-home and community-based services. CCP assists older adults to maintain their independence and providing cost effective alternatives to nursing home placement.
CCP who can apply for services: Anyone who meets all current eligibility requirements.
- Are 60 years old or older
- Are U.S. citizens or legal aliens
- Are residents of Illinois
- Have non-exempt assets of $17,500 or less
Community Care Systems, Inc. can provide non-medial support by supervised Homecare Aides who have received specialized training in the provision of in-Home Services. We can provide such services as assistance with personal care activities, routine house cleaning, laundry, meal preparation, companion & respite care, essential errands and escort, remind participant to take their mediations and relief for family member & caregivers who work during the day.
